  Reason for Request:
Repair: Sending an item in for repair
Advance Replacement Credit: Use this option if you have purchased and received a replacement part prior to returning the original part. The Sales Order # or your Purchase Order # for the advanced order must be provided.
Regular Credit: Didn’t need, defective, wrong order or just returning. Must enter the original Sales Order# / PO# / Invoice# from the original purchase.
